Disability
Pension
You
will be considered totally and permanently disabled if the Board of Trustees
finds, on the basis of medical evidence, that your disability will be
permanent and continous during the remainder of your life and you are
unable to work in any gainful employment.

Have
at least 15 Pension Credits; and
-
Worked
in Covered Employment for at least three months in one of last two
calender years prior to the year in which you became disabled; and

Proof
of your disability is certified by a physician and it is approved
by the Trustees
